ABSTRACT

Introduction: The glucagon-like peptide-1 [GLP-1], is a neuropeptide and an incretin that helps regulate glucose and insulin concentrations. The effects of acute aerobic exercise however on the response of GLP-1 are unclear. The purpose of this study was to investigate the effect of acute aerobic exercise on glucagon -like peptide-1 [GLP-1] serum levels, glucose and insulin in women with type II diabetes mellitus


Materials and Methods: In this study, 20 patients with type II diabetes were enrolled in two groups [experimental and control]. The experimental group underwent aerobic training one times [running on treadmill] with moderate intensity 65-75% HR max to exhaustion. Blood samples before exercise, immediately after and 24 hours after exercise were collected for measuring GLP-1, glucose and Insulin


Results: Results showed that none of sampling time effects on GLP-1, glucose and insulin were significant [p>0.05]. also, Pearson correlation showed that was significant relationship between GLP-1 changes levels and glucose [p<0.05]


Conclusion: Results indicate that in women with type II diabetes, acute aerobic exercise, with moderate intensity to exhaustion immediately and 24 hours after exercise has no effect on serum levels of GLP-1, glucose and insulin

ABSTRACT

Introduction: This study aimed to predict symptoms of depression based on cognitive flexibility, rumination and mindfulness of students


Method: This design of current study was of descriptive correlational type. The study population consisted of all students enrolled in 2014-2015 school year in Tabriz Azad University, among which 180 were selected by random cluster sampling. Subjects completed cognitive flexibility Inventory [CFI], the Beck Depression Inventory 2 [BDI-II], rumination [RRS] and the Freiburg Mindfulness Inventory. Multiple regression analysis was used to analyze the data


Results: The results showed that there is a significant negative correlation between cognitive flexibility and its subsets with depression [P<0.001], and there was a significant positive correlation between rumination and depression [P<0.001]. The relationship between mindfulness and depression was significant and negative [P<0.001]. The results of regression analysis showed that these variables explain 57% of depression


Discussion: The results of this study have therapeutic implications based on Cognitive Flexibility Training through cognitive behavioral therapy, as well as Mindfulness Training, on rumination reduction and depression symptoms

ABSTRACT

Introduction: due to the harsh conditions of employment, nurses face with many stressful factors that can affect their physical and mental health; as well as, their employment conditions. The main objective of this study was to evaluate the effectiveness of positive thinking training on hardiness, resiliency, and job burnout


Method: this was a quasi-experimental study. The samples were randomly selected from female Nurses employed in the governmental hospitals in Quchan, Iran. Then they were divided into two experimental [n=20] and control groups [n=20]. Data were collected using Burnout Inventory [MBI] and Maslach and Jackson and Connor- Davidson Resilience Scale [CD-RISC] and Kobasa hardiness questionnaire. Positive thinking training was administered over 8 sessions. Data were analyzed using SPSS- 16 software


Results: the results of analysis of covariance showed that there were significant differences in the levels of burnout in nurses based on their age, marital status, working conditions, and their type of working contract [P<0.001]. The results also showed that positive thinking training was effective on hardiness, resiliency, and job burnout [P<0.001]. Intervention resulted in the increased hardiness, resilience, and reduced burnout in the nursing staff [P<0.001]


Conclusion: according to the nurses, psychological interventions aimed at increasing hardiness and resiliency can reduce their burnout and improved job satisfaction and quality of service achieved

ABSTRACT

Celiac is an autoimmune disease that is characterized by an immune-system-related damage in the intestinal tissue after consumption of gluten. There is not any general agreement for gold standard. The Purpose of this study was the evaluation of specificity and sensitivity of anti-endomysial [EMA] and anti-tissue transglutaminase [TTG] Serological test compared to small-bowel biopsy. In the cross sectional study, we took blood specimen from 1825 patients with gastrointestinal disease. All the samples were tested by TTG and EMA kits using ELISA. The patients were studied in two groups. First, the individuals whom their serologic test was positive but their tissue condition was normal and second, those with positive serologic test with pathologic tissue results that show they have celiac disorder. The mean of EMA and TTG shows that the level of antibodies in group 2 is significantly higher than that of the first group [P <0.001]. There is positive correlation between modified marsh criteria of small-bowel biopsy and the two tests. The Sensitivity of EMA and TTG tests for celiac diagnosis is 92%. The specificity of EMA, TTG tests are 100% and 98.5%, respectively. EMA-IgA serology with cut-off point of more than 66 together with TTG-IgA serology with cut-off point of above 30 can be helpful to distinguish a wide range of patients who need small-bowel biopsy

ABSTRACT

All over the world, depression is a serious and debilitating public health problem with high prevalence. Medications significantly reduce the depressive symptoms and mortality. However, psychosocial interventions increase efficacy of medication. This study aimed to study the effects of dialectical behavior therapy skills on depressive symptoms and suicidal ideation in depressed patients. Our study design was a quasi-experimental and participants were selected based on volunteer sampling. 18 depressed inpatients were assigned in experimental [n=9] and wait-list control [n=9] groups. Patients were assessed by Beck Depression Inventory [BDI] and Reasons for Living [RFL] Scale before and after training. Dialectical behavior therapy skills training were performed individual weekly format and lasted for over 6 month and 20 sessions. Results of ANCOVA showed that skills training were effective in reducing depressive symptoms. Experimental group had a statistical significance in depression [P=0.001] and suicidal ideation [P=0.008] scores. Augmentation of medication with dialectical behavior therapy improves effectiveness of treatment of depressive symptoms and suicidal ideation. Experimental group was superior to control group in reducing symptoms. These skills can be used for the depressed inpatients

ABSTRACT

Congenital unilateral absence of pectoralis muscles in combination with ipsilateral brachysyndactyly is the most acceptable definition of Poland syndrome. This very rare syndrome has been reported in association with various malignancies and deformities including cardiovascular anomalies. To our knowledge, pulmonary stenosis associated with Poland's syndrome has not been described previously in literature, and there are a few articles about atrial septal defect [ASD] in combination with this syndrome either. We present the first report of Poland syndrome associated with pulmonary valve stenosis and another case of Poland syndrome with ASD. We discuss common etiologic factors of this rare syndrome. Case one: The first patient is a 23 month old boy with Poland syndrome presentation such as absence of pectoralis major of right hemi-thorax, nipple and subcutaneous hypoplasia and ipsilateral brachysyndactyly. On physical examination, we found a systolic ejection sound in auscultation so that a complete cardiac evaluation was performed. Pulmonary stenosis with 7[mm] diameter was detected by Color-Doppler echocardiography. The second patient is a 5 year old girl presenting typical Poland syndrome clinical manifestation. She has congenital right pectoralis major agenesis, Right upper position of nipple and areola was compared to left hemi-thorax, and an ipsilateral hypoplastic hand with syndactyly and short fingers. The 2-dimentional echocardiogram revealed that she is suffering from an Ostium Secundum ASD. Although Poland syndrome is a rare congenital one, it is associated with numerous anomalies and malignancies in the reported articles. The two mentioned cases are the samples of cardiac anomalies associated with this very unique syndrome. According to critical situation of cardiovascular problems, performing a complete cardiovascular evaluation in addition to common medical-surgical treatments seems to be essential for patients with Poland syndrome

ABSTRACT

Background: Skin thickness has an important role in aesthetic results of rhinoplasty. Currently, some surgeons employ defatting technique to reduce subcutaneous tissues and decrease the thickness of skin during rhinoplasty


Objective: The aim of this study was to compare the nasal tip skin thickness with and without nasal tip skin defatting technique and to further evaluate the long term effects of this technique on rhinoplasty patients by sonography


Methods: In a case-control study, 111 rhinoplasty candidates were evaluated at a university referral hospital in northern Iran during January to August 2010. Before operation, all patients were investigated through physical exams. Later, the skin thickness of the tip and supra tip were measured by sonography. Patients with moderate to thick nasal skin were randomly allocated to case and control groups for defatting. Sonography results of 44 patients, obtained one and 12 months after the surgery, were compared with those found in two groups before the surgery using Wilcoxon test and Repeat Measure test


Findings: A total of 111 patients with mean age of 25.1 +/- 7.6 years including 16 [14.4%] men and 95 [86.6%] women were enrolled. Based on physical examinations, the skin type was thin in 20 [18.9%], moderate in 68 [59.4%], and thick in 23 cases [21.7%]. Considering the sonography results, the skin type was thin in 7 [6.8%], moderate in 83 [80.6%], and thick in 13 cases [12.6%]. There was no significant difference in the thickness of tip and supra tip skin between defatted cases and control group measured before surgery and within the follow-up period


Conclusion: Based on results found through this study, Sonography is a precise instrument in assessing the nasal skin thickness before rhinoplasty. Defatting technique was shown to be of no effect on reduction of skin thickness of tip and supra tip in moderate or thick skins following rhinoplasty

ABSTRACT

Background: Studies have shown that the morphine can affect several neurotransmitter systems in the brain and impair memory retrieval. Moreover, it has been reported that the hydroalcoholic extract of Salvia officinalis can potentiate memory retention. Therefore, this study was conducted to evaluate the effect of hydroalcoholic extract of salvia officinalis on memory impaired by post-training administration of morphine using a passive avoidance paradigm in rat


Materials and Methods: In this study, rats [220-250g] were trained in a step through type passive avoidance task [1.5 mA shock for 3 seconds]. Immediately after training, the animals received systemic injections of morphine [2.5, 5 and 7.5 mg/kg, s.c.] or Salvia officinalis [20, 40 and 60 mg/kg, s.c.]. Another group received Salvia officinalis [20 mg/kg, s.c.] before training and morphine [7.5 mg/kg, s.c.] after training. All animals were tested 24 h after training to measure the step-through latency


Results: Findings indicated that post-training subcutaneous administration of morphine [5 and 7.5 mg/kg, s.c.] and Salvia officinalis [40 and 60 mg/kg, s.c.] impaired and enhanced memory retrieval, respectively in animals on the test day. Moreover, a systemic injection of Salvia officinalis [20 mg/kg, s.c.] before training restored the memory impairment induced by post-training morphine [7.5 mg/kg, s.c.]


Conclusion: It seems that hydroalcoholic extract of Salvia officinalis can prevent the morphine-induced impairment of memory

He was ultimately diagnosed with rare presentation of pericardial cyst .His echocardiographic data and Chest CT scan data were unique because most pericardial cysts are seen in right side of the heart unlike our patient who had suffered from large left sided form of the disease which is very rare

ABSTRACT

This is a single institute retrospective study [from 2007 to 2008] to evaluate how patient related factors [age, weight] and type of anatomical location of ventricular septal defect [VSD] could affect the outcome of surgery. Patients with any diagnosis who had undergone ventricular septal defect repair [a total of 25 patients] from 2007 till early 2008 at our center were evaluated retrospectively for factors which might be responsible for developing residual ventricular septal defects and heart block. Data were analyzed through univariate and multivariate analysis. There was 2 deaths among 252 patients [0.8%]. The incidence of postoperative residual ventricular septal defect was 28.2% +/- 2.8* [71 of 252], but only 3 of them [4.2%] needed reoperation. Neither patch material [p=0.572], nor type [p=0.349] or size [p=0.599] of ventricular septal defect had any effect on this complication. The mean age and weight of patients who had residual ventricular septal defect compared to those who did not were not significantly different, although they were somewhat lower [4.7 +/- 0.7 vs. 5.2 +/- 0.4 years, p=0.537; and 15.4 +/- 1.7 vs. 17.9 +/- 1.1 kg, p=0.222, respectively]. There were five patients [2.05] with postoperative complete heart block [CHB] and again this was independent of the patients' age, weight and surgical approach [transatrial or transventricular]. Patients with history of previous Blalock-Taussig [BT] shunt proved to have postoperative bleeding more commonly [13%, 6 of 46 patients] than patients who had not [3.4%, or 7 of 206 patients, p=0.009]. Also in patients with a history of BT shunt compared to those without it, postoperative pericardial effusion [6.5% versus 1.5%, P=0.04] and pneumonia [4.35 versus 0.5%, P=0.025] were more common. It seems that for VSD repair, there are no limitations such as wight or age to proceed with the definitive surgery. Also the incidence of complications is independent of the type of anomaly or approaches for closing the defect. Finally, BT shunt has its own complications which are neither rare nor minor, so it is advisable to proceed with the definitive surgery at the first time to avoid the complications associated with BT shunt

ABSTRACT

Magnetic resonance imaging [MRI] is a noninvasive and valuable method in the diagnosis of Multiple Sclerosis [MS]. Compared with other modalities, the sensitivity of MRI for detection of the lesion increases using magnetization transfer [MT] and delayed imaging. Our aim was to compare the two methods in detecting MS lesions. In this double-blind clinical trial, twenty-one patients with the definite diagnosis of MS referred to Poursina Hospital, Rasht were included. Two radiologists evaluated all the images. First, images without contrast were conducted, then 0.1 mmol/kg contrast material [Dotarem, single dose] was injected and after 30 minutes, T1W and MT images were obtained. Seventy-two hours later, T1W images were obtained immediately after injection of 0.2 mmol/kg contrast material [double dose]. The data were analyzed using Fisher's and McNemar tests by SPSS for Windows. Delayed magnetization transfer showed 44 enhanced lesions using MT [69.84%] and 29 lesions using T1 [46.03%]. In addition, the number of enhanced lesions in the delayed method were significantly more than those in the immediate method [p value=0.003]. The use of single dose in combination with MT and delayed images after 20-30 minutes enables us to detect more enhanced lesions

ABSTRACT

Numerous workers are engaged in processing of tobacco in Iran and therefore, their exposures to tobacco dust are likely to be high. The aim of this study was to evaluate the workers' exposure to tobacco dust. Total dusts were collected within the workers' breathing zone using a Personal Air Sampler [PAS] and respirable dusts were measured using a real time monitor [Micro Dust Pro] with a particle size adapter in different parts of the factory. To weigh the filters a microbalance accurate to 5 decimal places was used. The mean total tobacco dust concentrations for personal exposure near breathing zone in this study was 9.32 mgm-3 that was lower than TLV recommended by American Conference of Governmental Industrial Hygienists [ACGIH]. In contrast the mean respirable dust concentrations was 3.28 mgm-3 which is higher than Threshold Limit Value [TLV]. Dust control methods such as good maintenance of existent dust emission control systems, insulation of dust sources and designing suitable local exhaust ventilation should be applied to maintain workers' health

ABSTRACT

Osteoarthritis [OA] is the most common disease of the joints and also a main physical disorder in the elderly population. One third of people older than 65 years-old suffer from OA. It is predicted that by 2020, OA is the most frequently reported chronic condition among the elderly. The treatment includes drugs, physical therapy, and surgery. Massage therapy is an alternative treatment option to relieve the symptoms, which is not usually used for knee OA. As there are very few trials in the literature regarding knee OA treatment, this study is set to examine the effects of Swedish massage on knee OA. In a randomized clinical controlled trial on the effect of massage therapy of the knee OA, 46 female adults [at least 35 years of age] with knee OA were enrolled in the study. The patients were randomized in one of the two groups: the experimental treatment group receiving the standard treatment [ultrasound, infrared and transcutaneous electrical nerve stimulation] with Swedish massage and the control group who received the standard treatment only. Before and after the intervention, the pain and the quality of life were evaluated using visual analogue scale [VAS] and the Western Ontario and McMaster Universities [WOMAC] Osteoarthritis index. There were significant differences in the pain level and quality of life before and after the intervention [P<0.05]. The findings showed significant improvement in the mean WOMAC and VAS scores in the two groups [P<0.001]. The changes in VAS and WOMAC scores were compared between the two groups which demonstrated significant difference between the two groups [P<0.001]. Massage therapy is efficacious in the treatment of knee OA. It is recommended to combine this method with the other physiotherapy methods for knee OA

ABSTRACT

Immunization against diphtheria, tetanus and pertussis [DTP] has long been applied in Iran using whole cell vaccine. Despite the role of whole cell DTP [DTwP] vaccine in reduction of mortality as a result of disastrous diseases such as diphtheria, tetanus, and pertussis, serious local and systemic complications have been attributed to these vaccines. This study was performed to determine the complications of DTwP vaccine in infants attending some of the health centers of Tehran in 2006-2007. In this prospective study, 330 infants were injected with DTwP vaccine manufactured by Razi Institute of Iran. All subjects received DTwP vaccine at 2, 4, and 6 months of age following the national vaccination schedule of Iran. Reactogenicity was assessed by the parents for 7 days post-vaccination using diary cards. Of the 279 infants who completed the vaccination study, pain was the most frequent local reaction after the primary vaccination [68.1-75.3%]. The mean diameters of the redness and swelling at first day post-vaccination were 2.81 +/- 6.91 and 2.60 +/- 7.93 mm in the first dose, 2.40 +/- 6.25 and 1.94 +/- 5.74 mm in the second dose and 2.24 +/- 5.66 and 2.16 +/- 6.03 in the third dose, respectively. Fever [axillary temperature > 37.5°C] was the most frequently reported systemic reaction during the primary vaccination [53.8-58.8%]. All systemic reactions observed after each dose were either reduced or completely disappeared during a week. The high incident of complications observed following vaccination with this cellular triple vaccine may be related to the formulation or the bacterial cell fragments used in vaccine production

ABSTRACT

Disability Weights [DWs] are main components for computing summary measure of population health [SMPH] and economic studies. They are specific for each community, but there are no previous studies in Iran. In this study, we investigated the feasibility of health state valuation [HSV] in Iranian population. Twelve cardiologists in 3 sessions of expert panels, defined 25 states, related to cardiovascular diseases [3 major and 22 specific diseases]. From January to March 2008, 80 persons in 4 groups including: physicians, patients, patients' families and general publics [each group 20], were interviewed and valuated the states, using visual analogue scale [VAS] method. SPSS[Registered] 15 for window[Registered] [SPSS Corporation, Chicago, Illinois] was used for statistical analysis. Data showed that the defined health states had various severities. All the 4 groups ranked the "3 major-diseases" and "very-mild" and "very-severe" states, similarly. Non-physicians were not able to differentiate among "valvular-diseases" and "pacemakers" properly. The reliability of responses was acceptable. VAS is an appropriate and reliable method for HSV in Iranian population. Non-physicians' opinions can be consider in major cardiac diseases. Valuation of more specific situations must perform by physicians

ABSTRACT

Aurein 1/2 is a 13-residue peptide with a vast antimicrobial and anticancer activity. Two- dimensional NMR spectroscopy of peptide solubilized in the 70% TFE [2, 2, 2- Trifluoroethanol] indicated an alpha-helical conformation. The mechanism of its action is not yet fully recognized. This study was designed to improve the antimicrobial activity and relationship between subsequence-activity in Aurein 1/2 and its analoges. Analogs of this peptide were designed and synthesized. The G1F3/RW and F3W analogs and retro - analog were synthesized with solide phase and purified via HPLC and lyophilized. These analogs were assayed by several methods: amino acid analysis, HPLC, and electrospray mass spectrometry. Then antimicrobial activity of the peptides was assessed by using the standard microdilution susceptibility test. The data demonstrated that G1F3/RW analog had a higher activity and results of test figure of minimum inhibitory concentration for F3W analog had three levels. But the native, F3W analog and retro-analog was inactive. The higher activity of G1F3/RW in compare to F3W may be related to the positive charge of Arg that leading stronger interaction with the negative charges on the membrane surface. The result showed that reversed direction of aurein 1/2 significantly effects on activity of the peptide. It is also suggested inactivation of reto-analog amino acid type, position and size should be cautious for peptides designed as drug because it may be effect to control demerization and maintenance of antimicrobial activity of the peptide

ABSTRACT

It has been proposed that a narrow intercondylar notch may increase the risk of anterior cruciate ligament [ACL] injury but the data are conflicting. We performed this cross-sectional study to investigate if a narrow intercondylar notch width is a risk factor for ACL tears. All adult patients with knee problems, who were referred to the MRI department of Poursina Hospital, Rasht, Iran, from October 2006 to October 2007, were included in this study. Axial and longitudinal MRI were performed using a 1-T Phillips machine with the patient's knee in an extended position. In all patients, the femoral notch and the distal condylar width were measured. Cases with normal ACL were used as control and patients with a complete or incomplete tear of ACL were chosen as case group. Because of the effect of osteoarthritis in decreasing the intercondylar notch index, cases with obvious osteoarthritis were not included in the study. Independent sample Student's t test was used to compare the means. 328 patients were enrolled in the study. The age range was 18-72 years. We found no significant difference in the mean notch width index [NWI] in patients with [0.296] and without [0.298] an ACL tear [P>0.05]. In addition, there was no significant difference in the frequency of ACL tears in patients with and without critical notch stenosis [P>0.05]. We did not find a relationship between narrow intercondylar notch width and ACL tears and we do not recommend a knee MRI to predict the probability of ACL injury

A2B negative is one of rare subgroups of ABO blood group system. Herein, we report a 59-year-old male patient who was candidate for coronary artery bypass grafting surgery [CABG] due to coronary artery stenosis. The patient`s blood group was reported as AB negative in routine laboratory, and because of doubtful result, a complementary test confirmed his blood group as A2B negative. The consultant hematologist recommended reserving either negative A2B packed red blood cell [PRBC] or if unavailable O- blood group. After induction of anesthesia three units of patient's own blood were collected and replaced by colloid solution. The patient underwent CABG with hypothermic cardiopulmonary bypass. The collected autologous blood units were transfused at the end of operation. The patient received one unit of A2B- homologous PRBC in the postoperative period, and was discharged without any reaction to transfusion

ABSTRACT

High concentrations of respirable particles may cause high incidence of respiratory diseases and mortality in public populations and in particular in public transportation workers. Epidemiological exposure studies showed that respirable particles that emitted from traffic sources have more potential effects on more incidence of hospitalization or mortality caused by cardiovascular diseases. In this study personal exposures of bus drivers to respirable particles [PM10] and factors that caused high personal exposure were studied in Sari city. In this study, a calibrated real time monitor [MicroDust Pro] with a personal sampling pump with 2 lit/min flow rate were used to measure Sari's bus drivers personal exposure to PM10 concentrations on 3 main routs. This study was carried out during a one calendar year and all possible effective factors on personal exposure were recorded in a questionnaire. The mean personal exposures to PM10 concentrations among bus drivers were varied in different months. Exposure to PM10 concentrations were higher in autumn and winter and increased to 300 micro gm[-3] in some days. Personal exposures to these particles were increased in the evening and on high traffic flow routs. Other factors such as rainy weather and temperature, number of passengers, bus' model year, picking up and dropping off the passengers were the most effectiveg factors on driver's personal exposures. Bus drivers' exposure to respirable particles PM10 in Sari was varied in different time. It was due to entrance of outdoor particulate air pollutants into the bus during the picking up and getting off the passengers and resuspension of settled fine particles

ABSTRACT

As depression and anxiety can have profound negative effects on the functional status, quality of life, duration of hospitalization and even medical outcome of cancer patients, evaluation and treatment of these disorders are important. This was a descriptive study to assess the prevalence of anxiety and depression in cancer patients hospitalized at Isfahan Sayyed-o-shohada hospital. A toal number of 173 hospitalized patients were enrolled. The Beck Depression Inventory [BDI] and Zung Self-rating Anxiety Scale [ZSAS] were applied to measure depression and anxiety respectively. Data were analyzed using chi-squar test running SPSS-ll soft-ware. Prevalence of anxiety and depression was found to be 18.5% and 32.9% respectively. Anxiety had a higher prevalence among women [P<0.05]. Patients in the age group of 50-65 years old had the highest rate of anxiety [p<0.05]. Age or gender had no correlation with depression. Type of malignancy, duration of treatment and treatment method were not correlated with prevalence of depression and anxiety
